Maybe it’s on the road as you travel… or maybe it’s in the corner of your local tavern, smiling at the staff. Ever noticed that one tavern regular who seems... too regular? Horror in the Mundane

This idea came to me after watching Buffy the Vampire Slayer S6E12, Doublemeat Palace. In this episode Buffy, in desperate need of income, takes a job at the local fast food restaurant Doublemeat Palace. Buffy takes a job at the local fast-food joint, Doublemeat Palace. The real horror? It’s not the food service grind – employees are disappearing, and the Slayer suspects something sinister lurking behind the counter.

Doublemeat Palace’s Hidden Horror




But the real threat isn’t the manager or strange meat… it’s the regular customer Buffy calls 'Wig Lady.' On the surface, she’s just a quiet elderly patron. But when confronted, the truth literally bursts forth.

The creature that tears free from her skull is like a nightmare – a twisted blend of eel, parasite, and predator, with the unsettling gaze of the woman it once wore, like something from the mind of HR Geiger.



This creature was equally horrific in how it blended into an urban setting. It disguised itself as a seemingly innocent patron, and used that disguise to prey on its food source. That creeping dread throughout the episode, knowing that something was hiding in plain sight but not knowing what or who made me realize this would be the perfect monster to build.

Crafting the Skinwretch

Looking at this monster, I quickly identified what inspired me the most about it. This lamprey-like creature was a solitary urban predator that blends into crowds, ambushing its prey. In a system like Dungeons & Dragons, this would be perfect for a low to mid-tier party, putting it somewhere between a CR 5-8 range mechanically speaking.

Another element I wanted to really encompass was the ambush aspects of this creature. From its ability to remain hidden in plain sight, only being found when investigated and being able to seemingly catch its prey off guard was excellent.

I also loved the paralytic spray, especially how it slowly took effect. It’s not an instant death – the spray creeps through the body, inch by inch, leaving prey helpless to watch the creature approach.

Lore and Narrative – The Infected Host

My version of this creature, called the Skinwretch leans harder into infection and body horror. Imagine something lurking in the very food you eat, waiting to hatch and take over your body. Once consumed, it devours the host’s brain and takes over, growing into its horrifying form. The body around it is dead tissue, slowly decaying, but the parasite keeps it animate and seemingly alive through unnatural means.

After fully grown, a Skinwretch frequents high traffic areas such as taverns, waiting to find isolated prey it can eat. Eventually, after it has eaten enough, it will shed its disguise and find a suitable place to lay its eggs, the horrific cycle starting anew.

The Tavern of No Return

I could imagine this creature being the antagonist of a short adventure. There’s a tavern in town that’s never empty – the food is too good, the drinks flow freely, and the owner seems to have hit the jackpot. But something’s wrong. Workers disappear. Guests check in… and never leave. At first, the party suspects the owner. But one face stands out in the crowd – the sweet old regular who never misses a meal.
